You’ve been considering egg donation for some time now, and are ready to begin the application process, but you’ve also heard that you need to gather family medical history to include with your application materials. What information do you need to gather?
In addition to answering a variety of questions about your personal background in narrative format, submitting photographs of yourself, and providing information about your academic and/or career endeavors, our egg donor application asks for a considerable amount of information about your family medical history, including the medical histories of your immediate family (parents, siblings) and your extended family (grandparents, aunts and uncles, cousins, etc).

Completing the family medical history section of your application form thoroughly and accurately provides hopeful parents (and their future offspring) with relevant medical information. You will be asked to describe the physical characteristics of your family members, as well as will be asked to provide medical information regarding your family members, including past and current medical conditions.
Below is an abbreviated list of the information you will need to gather in order to complete your egg donor application:
1. Current ages (or ages at death) of family members.
2. Physical characteristics of family members.
3. Medical conditions (past or current) of family members.
4. Personality or mental health concerns (past or current) of family members.
5. Cause of death, if family member is deceased.
Gathering up the comprehensive details about your family medical history may seem a bit daunting, but it’s vital information for hopeful parents and future offspring born through your donation. Further, and depending on the clinic where you cycle, you may be required to meet with a genetic counselor during your initial screening to talk more in-depth about your personal medical history and the history of your immediate and extended family members.
